If you usually receive repeat prescriptions from Eclipse Pharmacy and you are running low on your medication and are due to get a new supply, please fill in the following form.
Once we receive your request, we will pass on your details to your GP. Please give us 3-5 working days from the date of your request for us to receive back your prescription from the GP.
If you are not yet due, we will contact you by email to let you know when your medication is due.
